The company’s skilled nursing facilities offer medical services prescribed by physicians, licensed therapy services, nutrition services, social services, activities, and housekeeping and laundry services. Its licensed therapists provide physical, speech, respiratory, and occupational therapy for patients recovering from strokes, heart attacks, orthopedic conditions, neurological illnesses, or other illnesses, injuries, or disabilities. The company’s long-term care facilities offer care for Alzheimer’s patients in early, middle, and advanced stages of the disease, as well as specialized care and programs for persons with Alzheimer’s or related disorders; and managed care contract facilities that provides sub acute and other medical specialty services. It also operates assisted living centers, which offer personal care services and assistance with general activities of daily living, such as dressing, bathing, meal preparation, and medication management; owns or leases, and manages independent living centers that provide specially designed residential units for the active and ambulatory elderly, and offers various ancillary services for its residents, including restaurants, activity rooms, and social areas. In addition, the company offers health care programs that provide skilled services, which include infusion therapy, as well as wound care and physical, occupational, and speech therapies; operates pharmacies; provides management, accounting, financial, and insurance services; and leases its properties to third party operators. As of February 20, 2015, it operated 74 long-term health care centers with 9,462 beds; 36 homecare programs; 5 independent living centers; and 18 assisted living communities. The company was founded in 1971 and is based in Murfreesboro, Tennessee.
